Flex Plan Form
When a student requests to use testing accommodations in your class, they can be provided by the instructor or OA. The preferable method is for the instructor or GTA to administer the test with the given accommodations. This allows the student the same access to instructors or GTAs that their peers have when questions pertaining to the test arise. This also allows immediate access to the test for grading purposes.
Attendance Consideration
The Attendance Accommodation exists to provide justification for absences related to a student’s documented disability or health condition. In most cases these absences do not require a visit to the Medical Clinic or a doctor’s office. The student has been instructed to communicate with faculty in a timely manner when they need to use this accommodation. The Attendance Accommodation is not meant to exempt a student from an instructor’s attendance policy.

Steps To Complete Flex Plan Form
Login Into
Agree to Access Policy
Click on Flex Plan
Located under the Home menu.
Click View
This will open the Flex Plan Form.
Complete the List of Questions
Upload Syllabus
Instructors have the option to upload a syllabus to better help facilitate the accommodation based on your course policies.
Copy Form To Other Sections
Instructors have the option to copy the form to the same course or to other courses. Select the option that applies: Other Requests in the Same Course or Other Requests in My Courses.
Agree to the statement listed
Under Form Submission, check I agree to the statement listed and click Initiate and Request Review.
Accommodation Specialist Review
The student's assigned Accommodation Specialist will review the form and send it to the student.
